Clear checks and a score you can explain
Each note runs through pass or fail checks: signature timing, note times against billing, credentials, place of service, what the narrative has to show. The score adds them up.
The same note always gets the same score, and every flag points to the rule behind it.
Clinical checks
Clinical checks read each narrative the way a careful supervisor would. Is there enough detail? Is it copied from another note? Is the client's progress clear?
Every note gets a clinical score next to its score, with a short reason for each finding.
